BA.2.86 fears fizzle as other variants drive up hospitalizations, deaths

Concern over the highly evolved omicron subvariant BA.2.86 is easing as the first batch of preliminary studies on the virus suggests it may not be as immune evasive or dangerous as its numerous mutations suggest.

But the good news is tempered by the latest COVID-19 data, which shows increasing rates of hospitalizations, emergency department visits, and deaths—all driven by the current gang of circulating omicron subvariants, led in the US by EG.5, FL.1.5.1 and XBB.1.16.6 . No single variant is dominant globally, though EG.5 is on the rise.

In the US, hospitalizations are up nearly 16 percent since last week, and deaths have risen almost 18 percent in that time. Test positivity is also on a steep incline, according to the latest Centers for Disease Control and Prevention data .

A remarkably mutated coronavirus variant classified as BA.2.86 seized scientists' attention last week as it popped up in four countries, including the US.

So far, the overall risk posed by the new subvariant is unclear. It's possible it could lead to a new wave of infection; it's also possible (perhaps most likely) it could fizzle out completely. Scientists simply don't have enough information to know. But, what is very clear is that the current precipitous decline in coronavirus variant monitoring is extremely risky.

In a single week, BA.2.86 was detected in four different countries, but there are only six genetic sequences of the variant overall —three from Denmark, and one each from Israel, the UK, and the US (Michigan). The six detections suggest established international distribution and swift spread. It's likely that more cases will be identified. But, with such scant data, little else can be said of the variant's transmission or possible distribution.

    Over 230 people get puzzling neurological disorder in Peru; emergency declared

Over 230 people in Peru have developed a rare, paralyzing neurological disorder called Guillain-Barré Syndrome, leading government officials to declare a national emergency and the World Health Organization to send out a disease outbreak alert.

So far, four people have died from the disorder, which involves the immune system attacking peripheral nerves. It often starts with progressive muscle weakness and numbness that can lead to paralysis and, in about a quarter of the cases, the need for mechanical ventilation.

Peru—a country of over 34 million people—typically sees fewer than 20 suspected cases per month of Guillain-Barré Syndrome (pronounced ghee-yan bar-ray or abbreviated GBS). But, between June 10 and July 15, the country tallied 130 cases, including the four deaths, bringing the year's total to 231, the WHO reported Tuesday.

Equatorial Guinea's first-ever outbreak of Marburg virus —a relative of Ebola virus that causes similarly deadly hemorrhagic fever—is continuing to grow, spreading over a wide geographic area with potentially undetected chains of transmission, officials for the World Health Organization said.

As of Wednesday morning, officials in Equatorial Guinea had reported nine confirmed cases, with seven confirmed deaths across three provinces since early February.

"However, these three provinces are 150 kilometers apart, suggesting wider transmission of the virus," WHO's Director General Tedros Adhanom Ghebreyesus said in a press conference Wednesday .

A vaccine advisory group for the World Health Organization said Tuesday that, at this point, it does not recommend additional, let alone annual COVID-19 booster shots for people at low to medium risk of severe disease. It advised countries to focus on boosting those at high risk—including older people, pregnant people, and those with underlying medical conditions—every six to 12 months for the near- to mid-term.

The new advice contrasts with proposed plans by US Food and Drug Administration , which has suggested treating COVID-19 boosters like annual flu shots for the foreseeable future. That is, agency officials have floated the idea of offering updated formulations each fall, possibly to everyone, including the young and healthy.

In a viewpoint published last May in JAMA , the FDA's top vaccine regulator, Peter Marks, along with FDA Commissioner Robert Califf and Principal Deputy Commissioner Janet Woodcock, argued that annual COVID booster campaigns in the fall, ahead of winter waves of respiratory infections—such as flu, COVID-19, and RSV—would protect health care systems from becoming overwhelmed. And they specifically addressed the possibility of vaccinating those at low risk.

A group of independent, international researchers has released its full analysis of newly uncovered metagenomic data collected by the Chinese Centers for Disease Control and Prevention in January and February of 2020. The data closely links SARS-CoV-2 to the genetic tracks of wild animals, particularly raccoon dogs , sold at the Huanan Wholesale Seafood Market in Wuhan, China, the early epicenter of the COVID-19 pandemic, the group's analysis says.

The full analysis provides additional, compelling evidence that the pandemic coronavirus made its leap to humans through a natural spillover, with a wild animal at the market acting as an intermediate host between the virus' natural reservoir in horseshoe bats and humans. It was authored by 19 scientists, led by Michael Worobey, an evolutionary biologist at the University of Arizona; Kristian Andersen, a virologist at the Scripps Research Institute in California; and Florence Débarre, a theoretician who specializes in evolutionary biology at France's national research agency, CNRS.

Prior to the release of the full analysis late Monday, information on the findings was only made public through media reports and statements from the World Health Organization, which was briefed on the analysis last week. But, the raw metagenomic data behind the analysis is still not publicly available. It was briefly posted on a public genetic database called the Global Initiative on Sharing Avian Influenza Data (GISAID) as recently as earlier this month, and the international researchers were able to download it during that window of availability. But, administrators for the database quickly removed the data after its discovery, saying the removal was at the request of the submitter, a researcher at China CDC.

Newly obtained genetic data from the Chinese Center for Disease Control and Prevention (China CDC) links the pandemic coronavirus SARS-CoV-2 to animals—specifically raccoon dogs—at the Huanan Seafood Wholesale Market in Wuhan, where the earliest COVID-19 cases centered, a group of independent scientists told the World Health Organization this week.

The genetic data came from environmental swabs collected at the market by China CDC in January of 2020. The existence of these swabs was previously known, as was the fact that they were positive for SARS-CoV-2 genetic material. But in late January of this year, scientists at China CDC uploaded—and then later removed—additional genetic data from these swabs to a public genetic database called GISAID, the WHO said. That additional data, which had not been previously disclosed, indicates that the SARS-CoV-2-positive swabs also contained genetic material from humans and animals, particularly large amounts of genetic material that closely matches that of raccoon dogs.

Raccoon dogs—foxlike animals whose faces closely resemble those of raccoons—are known to be susceptible to SARS-CoV-2 infection and were known to be sold at the market.

On May 7, health officials in the UK reported a case of monkeypox in a person who had recently traveled to Nigeria. The case was very rare but not necessarily alarming; a small number of travel-related cases of monkeypox pop up now and then. The UK logged seven such cases between 2018 and 2021 . But this year, the cases kept coming.

By May 16, the UK had reported six additional cases, mostly unconnected, and all unrelated to travel, suggesting domestic transmission. On May 18, Portugal reported five confirmed cases and more than 20 suspected ones. The same day, health officials in Massachusetts reported the first US case . Spain, meanwhile, issued an outbreak alert after 23 people showed signs of the unusual infection. Cases in Italy and Sweden followed .

In the past, monkeypox transmission largely fizzled out on its own. Experts did not consider the virus to be easily transmissible. Still, the cases kept coming. By May 26, the multinational outbreak had exceeded 300 cases in over 20 countries. At the time, the US had only nine cases confirmed, but the Centers for Disease Control and Prevention announced that it presumed domestic community transmission was already underway. In early June, the global tally exceeded 1,300 from 31 countries, including 45 cases in the US.
